Who we are

Hiraya Dental Group is a dental practice in Azusa, California. This notice explains what happens to the information you give us through this website.

The call-back form

If you ask us to call you back, we collect your name, your phone number, the best time to call you and, if you choose to tell us, how you heard about us. We also note the page you sent it from, which of our ads brought you to it if you came from one, and the time you sent it.

We use it to call you back, coordinate your appointment, and keep a record of our communications. We will not send you texts, and we will not add you to any mailing list or use it for marketing.

Your call-back request is sent to Hiraya’s practice email so authorized staff can contact you and arrange an appointment. The website does not store the information you enter. Service providers supporting our website, email, telephone and appointment scheduling process information as needed to provide those services.

When you contact us or request a call-back, we use your information to respond to your inquiry, arrange appointments, and maintain records of our communications. We retain these communications for practice operations and recordkeeping, subject to applicable privacy laws. If you book a visit, information collected for your appointment becomes part of your patient record.

To request access to or deletion of your inquiry information, speak to the staff member who contacts you or, from October 10, 2026, call (909) 435-2397 and ask for the person responsible for privacy requests.

The opening-updates form

Until 26 September 2026, this website had a form where you could sign up for updates about our opening. It asked for your email address, and your first name and city if you chose to give them, and it recorded when you gave permission and the page you signed up from. It asked nothing about your dental health.

That form has closed, and the list of people who signed up has been deleted. We do not sell your information. We do not share it for advertising.

Please do not send us health information through this website.

Your information

You can call us at (909) 435-2397 and ask our Branch Manager to tell you what information we hold that you gave us through this website, or to delete it. We will review each request and respond according to applicable laws and our recordkeeping requirements. You do not need to give us a reason.

Cookies and tracking

This website does not use advertising cookies, and it has no analytics or tracking of its own. Cloudflare, the company that hosts the website, adds its own visit-counting script to the "page not found" page that appears when an address does not exist. We do not use a cookie banner because we do not set the kind of cookies that need one.

If that changes, we will update this notice before it changes.

Changes to this notice

If we change how we handle your information, we will update this page and change the date at the top.
